FORM NAME	USE THIS FORM TO...
	...provide听Proof of Immunization against measles, mumps, rubella, tetanus-diphtheria and meningitis prior to registering for classes. Required by Louisiana law of all first-time students born after 1956.
REQUEST FOR OFFICIAL COLLEGE TRANSCRIPT	...request official transcripts from other colleges or universities you have attended. Some institutions may charge a fee for this service, payable by the requesting student.
	...ensure sufficient funds will be provided for the student鈥檚 academic studies.
SEVIS Transfer to 橙子视频	...request approval from an International counselor to release information to transfer to 橙子视频, as required by the Student and Exchange Visitor听(SEVIS) Program.
	...update student's contact information should there be any changes to contact information already on file.
	...request letters of invitation to student's graduation ceremony.
CURRICULAR PRACTICAL TRAINING (CPT)	...request CPT authorization. The form must be completed and signed by the student and the student's academic advisor. The student must make an appointment with the SEVIS advisor to complete the CPT authorization in SEVIS. The top portion of this form should be typed.
LEAVE OF ABSENCE	听...request a leave of absence from the university.
PROGRAM EXTENSION REQUEST	...request a program extension if you will not graduate by the "Program End Date" listed on your I-20. Submit this form at least a month before the program end date listed on your I-20.
REDUCED COURSE LOAD听(RCL)	听...request a reduced course load (RCL) for one semester.听The form must be completed and signed by the student and the student's academic advisor. The student must make an appointment with the SEVIS advisor to complete the RCL authorization in SEVIS.
J-1 EXCHANGE VISITOR APPLICATION (Researchers and Scholars only)	...request approval for a J-1 researcher or scholar. The application packet must be completed by the sponsoring faculty/staff. For ULM Faculty and Staff use only.
